A chilling video has captured the moment alleged cartel members ran through a clinic in an attempt to assassinate two men in intensive care but left four people dead after a shoot out.

CCTV cameras at Culiacán Clinic Hospital in Sinaloa, Mexico showed staff running for their lives, hiding in offices and behind counters to shield themselves from the gun battle that had broke out in the facility last Thursday night (September 28). Alleged hitmen had entered the hospital to kill two men, aged 30 and 31, as they lay in intensive care after being shot a day earlier in Badiraguato.

Reports say there were three gunmen at the medical facility protecting the injured men when violence broke out. They could be seen running through the clinic shortly before one of them, a 24-year-old male, crashed to the ground after being shot in the head.

One of his accomplices appears to remove an object from him before fleeing. Four people died in the fighting including Dr Otniel Montoya, who died of a gunshot wound after he was caught in the crossfire shortly after starting his shift. He leaves behind a wife and two daughters, including one was born recently.

His family wrote on social media: "It's not fair that his life was taken away like that. He was working, carrying out his professional duties for strangers to come and take his life, just as they did. He has a family, a wife and two girls, one of them newborn. God give you eternal rest, cousin."

One of the hitmen who killed his own boss required medical attention when the fighting came to an end. It has been reported that he used a gun on himself after realising his fatal mistake, choosing to take his own life rather than face whatever gruesome punishment the cartel would've had in store for him.

"All the events occur precisely because an alleged execution was attempted inside and there was an exchange of shots in which unfortunately the doctor died," Culiacán police chief Gerardo Mérida said Friday.

Crime blog site El Blog del Narco reported that one of the gunmen initiated the shoutout after they confused Sinaloa Cartel territory leader as a hitman and accidentally killed his own boss. Sinaloa Cartel gunmen loyal to Los Chapitos – the sons of infamous drug lord Joaquín 'El Chapo' Guzmán – are thought to have been at the hospital guarding another man who was receiving treatment.

According to El Blog del Narco, that high-ranking leader was the elusive "El 22" – thought to have been the gangster responsible for running the cartel's operations in Guamúchil, Sinaloa, working under the sons of Joaquín 'El Chapo' Guzmán. He had visited the medical facility to pay the bill when the men tried to kill him.

The hitman who killed his own boss required medical attention after the fighting but turned the used a gun on himself after realising his fatal mistake. "That's the bad thing about not knowing the boss," a source said.

Authorities have not said if the two gunmen who were killed were the men who were hospitalised or with the gang of assassins. A woman traveled on her own to General Hospital and was treated for a gunshot wound she allegedly suffered at the clinic. Police are currently investigating this claim.
